Medi-Cal Transformation
We are pleased to announce that Medi-Cal members will soon have access to an enhanced range of services to ensure comprehensive and holistic care that addresses their physical, mental, and emotional well-being. This new development extends beyond traditional health care settings, taking a more proactive approach to support patients in all aspects of their lives.
We offer the following CalAIM services;
CalAIM – California Advancing and Innovating Medi-Cal
CalAIM is striving to help the state’s underserved populations with a new approach to health care that includes both the social and clinical aspects of the individual. Soteria is committed to support implementing CalAIM objectives in Los Angeles County. CalAIM is an expansion of Medi-Cal, and all costs of the program are covered by Medi-Cal or Medicaid. Soteria works with both these systems to provide our patients with the best solutions.
CalAIM services provided by Soteria include Enhanced Care Management and Community Support Services. Soteria also specializes in helping individuals and families to participate in these new programs.
Our Enhanced Care Management helps individuals and families with face-to-face help from specialists in solving homelessness, people in transition from long term facility care to home care, people who want to establish a place to live and care for themselves, people being released from prison, and women with pregnancy or postpartum difficulties. Our ECM services are for adults, children, and families who need assistance. We provide 13 Community Support Services from helping individuals with deposits to secure housing, to environmental remediation to reduce Asthma Symptoms. Other services include:
- Housing Transition Navigation Services,
- Housing Tenancy and Sustaining Services,
- Short‐term Post‐Hospitalization Housing,
- Medical Respite,
- Respite Services for Caregivers,
- Personal Care and Homemaker Services,
- Environmental Accessibility Adaptations (Home Modifications),
- Medically Tailored Meals and
- Medically Supportive Foods,
- Nursing Facility Transition/Diversion to Assisted Living Facilities,
- Nursing Facility Transition to a Home.
